Copyright Assignment and Waiver Form

**THIS IS AN ASSIGNMENT AND WAIVER OF LEGAL RIGHTS. READ IT CAREFULLY AND BE CERTAIN YOU UNDERSTAND IT BEFORE SIGNING**

I am an 18-year old participant or parent/guardian of the child I am submitting artwork on behalf of today ("the Participant") in this online form.

I hereby grant to Plaquemines Parish School Board (“PPSB”) the absolute right and permission to use the artwork produced by me/my child for the Holiday Card Art Contest (“the Artwork”).

I, the Participant or on behalf of the Participant, hereby convey, transfer, and irrevocably assign to PPSB all rights, title and interest in and to the Artwork, including but not limited to, its entire and exclusive copyright under the U.S. Copyright Act of 1976 and all applicable federal and state copyright laws. Such rights shall include:

the right to reproduce the Artwork;
the right to prepare derivative works based on the Artwork;
the right to distribute copies to the public of the Artwork;
the right to display the Artwork publicly;
the right to alter, exploit and make changes to the Artwork; and
any and all other privileges and rights to the Artwork attributed to a copyright owner.

On behalf of myself or the Participant, I further waive any rights the Participant may have with respect to the Artwork under Section 106A of the Visual Artists Rights Act of 1990 that are typically reserved to an owner of a lawfully copyrighted work, including but not limited to, the right to display the visual artwork publicly and to any and all applications in which either the attribution right, the integrity right, or the personal moral right may be implicated.

I hereby certify that I am the Participant or the parent or guardian of the above-named Participant and do hereby give my consent without reservation to the foregoing on behalf of myself or the Participant.

I further acknowledge that typing my name in this form constitutes my acknowledgement of the form and represents my signature.
