What is Transition ?


The Individual with Disabilities Education Act (IDEA), requires that transition services be included in every student's IEP beginning at age 16 or earlier, when appropriate.

IDEA(2004) defines transition as a coordinated set of activities witin a results-oriented process focused on improving the academic and functional achievement, movement from school to post-school activities, education, training, employment, independent living and/or community participation based on the individual's needs, preferences and interest. It also includes instruction, related services, community experience, the development of employment and other post-school adult livinng objectives, and when appropriate, acquisition of daily living skills and functional vocational evaluation.

The transition from high school to post-school options can be challenging for some students. Our goal is to prepare students to become productive citizens in their respective communities.

We must get our students to a point where they can move from one stage in life to the next. Transition planning is the process that will help students to move forward into the next stage in their life.
