Early Care Application for Teachers
If you are a certified teacher and interested in early care for your child or if your due date is approaching, it is very important that you contact us as soon as possible. There may be a waiting list. Many forms must be completed before your child can begin. Please make sure immunizations are up to date. We will need a copy of your child's immunization record with the state seal on it. We will also need a copy of your child's birth certificate and your teaching certification. South Plaquemines Children's Center Handbook and many of the forms needed for registration are attached below.

If your child is returning from the previous year or you are applying for the program, please print, fill out, scan, and email the forms below to [email protected]. If you have the capability, you can also fill and sign the documents electronically. The handbook is for your information only.
